How to Use the Personal Assessment Page (for Clients and Visitors)

If you’ve been directed to a Personal Assessment page, this guide shows you how to use it to submit a consultation and receive personalized recommendations.

Step 1: Open the Link

• Click the link provided by the brand, or visit the form embedded on their website.

• The Personal Assessment page should load correctly, with proper design, layout, colors, and branding.

• Depending on the configuration made by the owner, you will be able to see the page in English or Spanish:
  

Step 2: Start the Assessment

1. Select your gender by clicking either the Male or Female button
   This will take you to the Terms of Use screen.

Step 3: Confirm Your Age

• If you are under 18, click the “Exit” button.

• If you are 18 or older, accept the terms and continue. 

Step 4: Select Your Information

1. Select your areas of concern on the 3D model by clicking on any of the symbols according to your body area.

2. Make your selections according to the specific areas you need and click Add to my assessment. Click the Back button or the Left pointing arrow on top to navigate to the previous screen.

3. If you don't see the needed or desired selection listed, scroll down in the list to find the field to add a special area of concern, then click Add to my assessment to continue:

4. You will then see a summary of your selections. Keep clicking the different body areas on the left to add more concerns:

5. If you would like to remove a specific selection, click the "X" button on the target.

6. If you would like to add more concerns to your consultation, just click on any other body part selector on the right.

7. If you would like to add more concerns manually for the same body part, click on the "Other" field in the desired section, add it, and repeat the process.

8. If you are done with your selections, click Finish Personal Assessment; you will be redirected to the submission form:
   

9. Fill in the required contact fields:
   - First and Last Name
   - Email
   - Birth Date (You must be over 18, otherwise the system will not allow you to continue)
   - Phone Number
   ? Tip: Make sure your contact details are correct to avoid delays in follow-up.

10. Optionally, add any additional comments and upload photos for providing more detailed information.

11. If you would like to review the Privacy Policy, please click on TERMS OF SERVICE.

12. Tick the box at the consent statement after carefully reading and click Send.

Step 5: You’re All Set!

Your consultation has been submitted and will be reviewed by a specialist. Expect a personalized response from the provider soon.
